Hi Dawn Ta Glad to know you found the research helpful. When I did the research, the context was for patients who have experienced their first bone fracture, so there is a cause for them to meet Orthopedist. Then, the orthopedist will conduct a risk assessment & determine whether the patient had high chance of osteoporsis and whether DEXA scan is suggested. Indeed, I think some of the elements could turn into a simple self assessment questionnaire for patients to check their risk level, so that he/she could proactively ask the doctor.

Riding on your idea, I am thinking if there is a centralized system that could store the medical record of patients which doctors could refer to. Patients could grant the right to doctors to access and patients could also subscribe reminders for medication & exercises etc. And if those data could be used as support data for insurance companies for underwriting, calculating insurance premiums etc. This would create benefits to the patients, doctors and insurance companies.
