A Sustainable and scalable Project Plan For The Improvement Of Sexual & Reproductive Health In A River Island.

What specific problem(s) are you trying to address?

We are currently working in a village isolated by the river Tista and in that village both women and adolescent girls never uses a sanitary napkin. No one even knows about the problems they are facing. If they are having a problem that can be tolerated, they don’t have the courage to go to consult a doctor.This is a place gets affected by flood each and every year.By interviewing the village women we found several problems- They don’t have the commodities when they need. It may take to walk one and half hour to go to the nearest pharmacy.Well, even if they can they won’t because every guys will see them and criticize them.Adolescent girls feel afraid and ashamed to talk to anyone about their state.They don’t feel really comfortable to talk.

What are some of your unanswered questions about the problem(s) you are working to address?

1. How might we make them used to use a sanitary napkin for their own sexual and reproductive health? 2. How might we make them comfortable while they are telling a professional about their sexual and reproductive health with any hesitation? 3.How might we make the male members of the village think positive about this approach and will they be able to help the female willingly?

Explain Your Idea

Our idea is to provide commodities and sexual and reproductive health service to village women and adolescent within their comfort zone. 1. We will be placing a health booth inside the village which will be operated by a minimum educated young girl (villager). A computer will be provided in the booth by the power of solar panel. 2. All health services and commodities and awareness program will be conducted based on the booth center. The name will be #LIGHTHOUSE. The reason for this kind of naming is that we will have a positive mental support from the father which will be a lot. 3. Sanitary napkins will be provided from this booth. And to break the social enigma we will have a package to provide “mother and daughter” together. This will help to break the ice about talking about sexual health. 4. “The black curtain chamber” will be the most important addition in the booth. As women and adolescent girls feel uncomfortable to talk about their sexual and reproductive health in front of someone even if that someone is a doctor, we will be using a black curtain between them. As we will be providing e-medical services too, the operator will listen to the ‘teller’ and write that down by ‘a serial number’. Later ‘she’ can get the health advices by just providing her serial number. This will work as a “village manually automated” health services.

Name the three most important ways that your idea will address your identified problem(s).

1. Women will be able to open towards their adolescent girls about sexual and reproductive health. 2. Village women will be comfortable to talk about their health issues in the heath booth as the traditional way of curtaining is being kept. We aren't here to break the social enigma. We just need to provide them support however they feel comfortable. 3. Commodities will be easy to them. They will be interested to a new and easy offer.

How is your idea unique?

The place we will be working on will make the idea unique. Secondly, we made the solution by asking the women as “what if we give you this that way? Will you take it?” this type of question. By building the solution by our users makes the idea more unique. Well if we determine the probability of success of our idea then we can be more sure of it than the other organizational activities. The main reason is we are trying to make them comfortable while taking the service. The success depends on the users. As this is a solution from the user with automated and highly recommended service, we generally expect more success from it.

What are some outstanding concerns or questions that you have regarding your idea?

How will be the doctoral examination if we work according to the ‘black curtain chamber’? How might we create awareness among the adolescent to participate? As the women don’t work in the villages, they don’t earn. So how they will pay for the commodities? Even they have the money, will they be eager to spend for a commodity they are living without? How might we provide emergency service if the health booth is open only a fixed time in a day?

Who are your end users?

Our end users are adolescent girls, early married women and aged women. In the village we have less adolescent girls than married women because of early marriage. All the married women are housewives but they work in the field when the husband is sick or for any other reason. All of the users are uneducated, afraid of something new.

During the flood the whole village goes 4/5 feet under the water. This year it was drown for 12 days. To provide women sexual and reproductive health service continuously from the booth we will be required to build the booth in a comparably high place. Or we can just make the base higher that the surface. We will be needing a strong operator for a very urgent situation. As this step will be opposite from the traditional ways we may face conflict from cultural and social issues.

We are a non-profit organization aged six years. We have worked for child education, agricultural development and village women employment. In our team we have engineers, doctors, agriculturists, planners and doers obviously. This year one of our cofounder have learnt about Human Centered Design (HCD) to find the actual problems, build insights and design solutions by attending a workshop conducted by the Portfolio Manager of From then we started implementing HCD in our work. We works on river islands in the northern part of our country and that where we work. We are a very young team with so many practical experiences and team spirits.

How has your idea changed based on feedback?

Several things of our preliminary idea got shaken a lot when we went to the community "Char Bazra". 1. We planned that there would be a fixed term employee whose duty would be delivering sanitary napkins and other SRH equipment. But when we talked to our users about this, we found that they didn't want that. They preferred a system where they would collect the napkins in their convenient time. 2. Based on the feedbacks, we made a prototype. That is, we made a group of 10 houses and one woman who would be responsible for providing napkins and other equipments to the women of these 10 houses. As the houses were adherent to each other they were comfortable about this. 3. According to the IDEO feedback, we surveyed 50 women on "Why aren't they using sanitary napkins?" 20% respondents we got the answer that nobody uses it as they feel ashamed if someone sees them buying or carrying, 50% is too poor to afford, 17% said that they do not get it easily and the rest didn't give any answer.

Who will implement this idea?

In Spriha, we have 212 volunteers in our community and we have 14 core members who are always working for the projects. In our project co-ordination team we have engineering graduates, doctors, marketing experts and project management experts. We are doing full time works in Spriha. This team is the founding team and core team of our organization. For this idea we will need four staffs to run the project in one to three villages. For ten to twenty villages it will require at most five employees if we provide them bicycle.

Using a human-centered design approach, you may uncover insights that lead to small or foundational changes to your organization’s existing strategy or processes in order to unlock the potential of your idea. How would your organization go about making such changes?

Well we already discussed that last year one of our co- founders got a primary HCD training from personnel. From then we started practising human centered design in our work. Since 2011 we were doing social development works and from one year we were trying to make a design approach in our work. Though it is being tough sometimes to go ahead with a little resource and guidelines, we added Human Centered Design approach as one of our key values of our organization. Big decisions of our organizations are mainly made by the excutive committee members and co-founders.

What challenges do your end-users face? (1) What is the biggest challenge that your end-users face on a day-to-day, individual level? (2) What is the biggest systems-level challenge that affects your end-users?

1. Our end users face several challenges in their day to day life. As they are housewives and they do not earn, this makes a huge difference in their life. They have a very limited amount to spend for themselves. Some of them fell prey to domestic violences. They do not have a voice to talk about their sufferings. They need to obey their husbands and listen to their order as soon as possible otherwise they get scolded. This is a very normal situation for the women and girls in the island. 2. Women and girls in the island have a very little education or no education. Early marriage is a very common system level problem they face. They aren't meant to be inside home all day long.

Tell us about your vision for this project: (1) share one sentence about the impact you would like to see from this project in five years and (2) what is the biggest question you need to answer to get there?

IMPACT: BY 2022, we want to spread our program in 10,000 villages of our rural area as well as river islands, and want to serve 2 million women and girls directly through our programs. QUESTION : How can we make a model that would be successful to provide SRH services to 2 million women with minimum costs, maximum process efficiency, durability, strength and sustainable in the context of our conservative society?

What is it that most attracted you to Amplify instead of a more traditional funding model?

Amply is more designed funding model than the traditional. Sometimes only monetary fund isn't enough and we will need experts feedback and suggestions to run our project. Only spending money without a proper plan would be a complete waste. We must need a strong team with the most expertise experiencess gathered from around the around. The guideline is half of the fund as we believe.

  • We aim to implement our Amplify idea in support of displaced populations, but not in a refugee camp / temporary settlement.

I just opened an account in IDEO to ask you some questions. I have worked in Bangladesh in some projects. It will be great if you implement this idea right now. But I must say you some important features to give importance.
1. If you are thinking this idea as scalable you should build a low cost napkin manufacturing system too.

2. How will you measure the success of this project as your probable beneficiaries don't usually disclose the problems?

We are planning to make a deal with a well known ngo as they produce napkins at very low cost. We also have plan to build a system of our own but that will take a minimum number of beneficiaries like 100k

Our approach is human centered. We have several plans to build a healthy relationships with our end users.

thanks for your kind appreciations. i'm believing in them because our organization is running some other programs, like modifications in traditional agricultural methods, transforming housewives into skilled handicraft producers, vocational schools for school drop-outs . and having positive feedback. so i do believe with proper funding and guidance
our organization will be able to run this project with possible maximum efficiency.
