The library could be customized for the most common scenarios encountered by CHWs in the developing world (Pareto analysis will help!) and preferably equipped with voice search (see http://www.cs.berkeley.edu/~eomer/SphinxTiny/) for quick access to content.


SD cards with the video content could be snail-mailed to all known CHWs in the field.


Nokia (catch up with Sharp !) could develop a rugged, waterproof, solar powered phone with the content preloaded (rather like the OLPC laptop) for immediate deployment by CHWs .

I think this is the right method for distribution. Cellphones are already distributed, so the platform is in place. The only problem then is how to get the content onto the mobile devices...
