Tracking of vaccine form manufacture to the child in real time using Blockchain technology

UNICEF MENA regional office aims to apply Blockchain technology to track the vaccine from the manufacturer to the child. The vaccines are tokenised in public Blockchain and the movement of vaccines through different levels are managed using paper and mobile wallets and visualised in real time using mobile application.

Explain the innovation (2,500 characters)

The UNICEF SMART CHILD initiative is a solution to reach unreached children with potent vaccine and validate the process using blockchain technology. It will avoid stock-outs and effectively trace the vaccine from the manufacturer to the child. It will remove major delays and cost to manage the vaccine and will reduce stockout and vaccine wastage through real-time monitoring solutions and allow the use of other advanced technologies like radio-frequency identification (RFID) and the Internet of Things (IoT) to interact with the blockchain and improve vaccine management practices in the long run. The major focus of this initiative is to take the potent vaccine to the unimmunized children around the world through monitoring their immunization status using blockchain technology and identifying the individual defaulters to take immediate action. All the vaccine are tokenised in public blockchain and the tokenised vaccine are moved along the facilities in Blockchain. The vaccine will be managed in Blockchain and the stock will be monitored in real time using mobile which will reduce the stock out of vaccine for a greater extent. It also will monitor the vaccination status of the children using their unique identity. The children; especially from urban areas will be registed in Blockchain. The tokenised vaccine will be transferred to the respective child's wallet during vaccination session which will allow identifying defaulters in Blockchain irrespective of the location from where the children are being vaccinated. This tool is already implemented on a limited scale in Syria and Iraq where the real-time vaccine management and monitoring have been demonstrated. The irreversible records in blockchain have already been demonstrated in Blockchain. The paper wallets have been created and exported to mobile wallets and tested. As per the nature of Blockchian, no failures happened so far. The urbanised population are facing different challenges especially highly mobile therefore the children will not strict with once health facility to obtain services, therefore, it is not possible to identify the defaulters through a regular paper-based system. The registration of children in Blockchain during their first visit and monitoring their vaccination status continuously though mobile will eliminate this problem and will allow identifying unimmunized children irrespective of the place from where the chidren going for vaccination.

Currently, the tool is being used in the MENA region as a pilot in limited locations, but there is no limitation for adopting this tool in any part of the world as vaccine tokens are created in public blockchain. All the applications which are being used are open source.

The major focus of the tool is to support the government to visualise the vaccine stocks at different levels in real time in a decentralised approach. The vaccines are tokenised in public blockchain and managed and monitored using paper and mobile wallets. This will allows tracking of vaccine from manufacturer to child. The tool is being tested in some countries. Based on their initial experience the tool will be further improved to be able to scale in any part of the world with minimum efforts. We are working with a private company and building a more user-friendly decentralised application (DAps) on the top of the blockchain platform. The project is going to be in three phases and currently, we are in the first phase to monitor the vaccines from national levels to health facilities level in real time; however, the tool allows monitoring the vaccine at all levels even now and tests are being carried out at other levels at this phase.

How is your idea unique? (750 characters)

We have been working with different tools to support countries to manage their vaccine at different levels. Most of the government adopt a low scale excel based solution and some are using web-based solution. Lack of visibility of vaccine stock at all levels in real time through current tools is making stock out of the vaccines inevitable as none of the current tool giving the possibility of monitoring the vaccine in real-time at all levels. The blockchain-based solution allows tracking of vaccines in real time with minimum resources; just need a mobile phone to implement this tool at any levels. This is more secured, immutable, trusted, transparent but anonymous which gives more trust to the partners.

This whitepaper was prepared by MENA regional immunization team in consultation with UNICEF team of PD and SD. This concept was presented in the UNICEF WHO hub meeting and the feedback was obtained. There are some changes in the timeline, for example, monitoring individual children was in the third phase in this initiative; however, this feature is already available in our tool to be used by any country.

I would be worried about about the sustainability issues that this project presents. Additionally, blockchain technology is a rather new concept and people in many parts of the world may not know it. It would be important to see you address these issues in the project brief.

we are fortunate to have such technology which was developed by an unknown person. Even though it seems to be new it has proven its effectiveness over ten years now and all the supply chain institutes have started to work on it. Wallmart is monitoring even the origin of the vegetable and what a shame if we cannot monitor the origin of the vaccines. It is really a breakthrough for supply chain management and there are many open source applications are coming up. Many blockchains initiative will disappear as dot come bubble, but few will survive; therefore we need more ground level experience and testing to have a sustainable and scalable solution even after five years.

Fortune Mutiso  Even though blockchain technology seems to be new, it is growing very fast. I have seen in Kenya even a village level people using their mobile to transfer the money; therefore why our health workers cannot use mobile to transfer the vaccine. The Blockchain mobile decentralised applications are even simple and secure than all the other applications as there is no single point of failure. In addition, having a dedicated ID was never been simple as with blockchain with any mobile-based application and this blockchain based ID is global; even if the child moves from Kenya to Tanzania or even Somalia still the vaccination status can be monitored using the same blockchain application.

Hi Ashley Tillman 
Thanks for your interest knowing about this project. As we have already created vaccines as a token in the Etherium blockchain and having a compatible reliable mobile application both in iOS and Android operating system, there is no limitation in conducting pilot of rolling out to any countries at this stage. The tokenized vaccine can be managed and monitored at any levels starting from National up to an individual child, but we have limited this pilot now for monitoring the vaccine only up to facility level or district level from national levels to learn the challenges with this new technology. So far our experience is very positive.
The first learning is the scalability of this tool to any country at any levels due to the decentralised nature of Blockchain. Even a part of the country (either a region, district or even a city wants to use the tool to monitor the vaccine movement from their level to the facility or child, it is still possible by creating the required wallet in Ethereum blockchain to transfer these vaccines. It can be expanded to other levels at a later stage with minimum efforts.
Second, learning from this pilot is quick implementation. People need very minimum training in a short period of time for downloading the mobile application and exporting the paper wallet to mobile and start moving the vaccine from one level to another, and monitor the vaccine at any levels in real time. Some places we are training just using skype and people are ready to implement the tool. Another learning is able to monitor any number of sites in real time using a single mobile. This innovation needs very minimum resources. All the available centralised vaccine management tools need a huge investment such a trusted software, server, server maintenance, software maintenance, licence fee, service agreement, computers and training of people on instaling in servers, computers and mobile etc, but thanks for blockchain which has removed all these burdens. The only a mobile is required and all the other operations are the responsibility of the people running the nodes of the blockchain. We just need a mobile to connect with the blockchain and start using it the same as we use the cryptocurrencies. The current pilot is only to monitor the number of vaccine by names at different levels, but we are working on a second layer solution which will allow monitoring the batch numbers, manufactures, date of expiry and even the VVM status of the vaccine at different levels in real time. Hopefully, it will be available by the end of this year. The major advantage of this tool is cost-effectiveness for introducing or scaling to any levels. We don't need much funding for implementation as there is no cost for servers, computers and massive training. There is no training sessions or training manual available on how to install the whatsup or viber and it is the same. People install the application and start using. The whole scope of "SMART CHILD" initiative is huge as I explained in my whitepaper such as using IoT and machine learning to integrate with blockchain which will allow monitoring temperature of vaccine and all the other attributes in real time from manufacture to the child. This is just a beginning but the full scope of this project is huge and it will allow monitoring of vaccine from manufacture to child in real time with minimum efforts and resources.
