Reaching Mobile Children for Timely Vaccination through Web-Based Birth Registration

Explain the innovation (2,500 characters)

In urban slums of Bagladesh, EPI services are performed through different NGOs. Although, Bangladesh has achieved tremendous progress in vaccination coverage, the coverage is markedly lower in urban slums (68%) compared to rural area (84%).Temporal movement (moving to rural village during delivery, inter-slum movement, duration of living in urban slum), parent’s education and occupation, and pregnancy registration - are the factors associated with immunization coverage.Therefore, tracking unvaccinated children is the challenge for EPI services – as these children are not being registered in EPI systems. To address these challenges, This project will be carried out in 4 slums of Dhaka with population of 122,000 individuals living in 32,000 households.Palm based biometric application will be developed to register children at first immunization contact with details of contact information of including cell phone number. This system provides reminder message for each of the vaccination and tracks the children who remain non-vaccinated. For each visit information on immunization will be updated in child’s vaccination profile in the system. Palm based biometric solution has already been proven more successful and cost effective than only name based registration.To provide an inclusive delivery service to the mobile urban population, weekly satellite immunization session can be conducted from 6pm-9pm (which is now conducted in the mornings only) for working mothers to encourage them to bring their children to the immunization session.To reduce gender disparity, the app data will be analyzed automatically to find if there are any inequalities in immunization coverage. As a demand generation tool, raising awareness and knowledge sharing programmes within the slums to state the implications of each vaccine should be conducted.An uninterrupted monitoring system will be built within the NGO to keep track of the health workers and maintain caretaker quality of care reporting. While providing the mobiles to the workers, an app will be built within each mobile to track the service hours of each worker.If the system can reach the underserved children through bio-metric application, will help to reduce outbreaks of measles infection and improve primary health care service in urban slum. If proven successful, it can be applied to other urban population in GAVI countries.

At this initial stage, icddr,b is planing to lead the project without any association. However, it is open to collaborate with other international organization ( i.e. SwissTPH) or any local NGOs in future, as required.

How is your idea unique? (750 characters)

icddr,b is already conducting a GAVI funded project titled in collaboration with SwissTPH which particularly focuses on immunization coverage in urban slums in Bangladesh and its associated factors. Thus it already has a comparative advantage over other organization to carry out this project successfully with the knowledge of the current project. Besides, we will introduce palm print based biometric solution rather than name based application which is easy to use, proven to show less error with a minimum cost.

This team of icddr,b work under the unit of Universal Health Coverage that harbors a diversified group of researchers having research expertise in the area of health system, health financing, health equity and ICT for health. In addition, there are research groups with specific research interest in urban health, surveillance system and evaluation of health service delivery. The team members include, Principal Investigator,Immunization Expert,Health Statistician,Data Management Supervisor,IT Specialist,Field Research Assistant (EPI Session observer),Data collectors (GIS).This team with the particular specializations will rightly be able to address the requirements of the project.
