What challenges or opportunities are you trying to address within the problem? (200 characters or less)

Challenges that are needing to be addressed are awareness, accessibility, and cost.

Osteoporosis is a condition that needs specialized care, similar to the specialized care given for other conditions such as anticoagulation therapy and diabetes management. My solution would fall in the post-fracture care journey at the follow up appointment with PCP after discharge for SNF and PT. At visit PCP discusses new diagnosis, provides recommendations and medication therapy. Often times, after this visit is when the care being provided to prevent repeat or future fractures fail. My solution or idea is to provide a facility with care providers that can provide them with the tools and skills to prevent future fractures and follow through of care. The PCP will provide information and instruction of exercises, diet, and supplementation. Skilled professionals in this facility will offer personal and group exercise classes that encourage exercise in a safe environment and instruction. Also, individual and group classes for shopping and cooking of foods that are vitamin d and calcium enriched foods. Support group meetings for open discussion of shared condition or fall, and Social workers to assist with insurance coverage questions and access. Creating a one stop location creates a since of community and builds trust and comfort. 

Who is your target end user and why will they be interested? (650 characters or less)

My target user would be health care organizations that cover and provide care in wide geographic areas.

What do/will you measure to know if your solution worked? (500 characters or less)

-Patient adherence to referral -Patient engagement -reduction in fracture rates

What is the current stage of development of your idea?

  • Blueprint: We are exploring the idea and gathering the inspiration and information we need to test it with real users.

If you were to become a Top Idea, would you want to actively participate in piloting your idea?

  • I want my idea piloted, but I’d prefer not to be involved in the pilot and prefer that the health system adopts my idea for piloting with assistance from the Challenge partner
