What challenges or opportunities are you trying to address within the problem? (200 characters or less)

Access to high-quality medically integrated health and fitness centre services is a major challenge among elderly patients in remote or underserved areas. What if we addressed these failures?

EldoPath’s goal is to provide medically integrated health and fitness centre services to healthcare entities like hospitals, correctional facilities, and tribal communities. In many healthcare systems, elderly patients’ full healthcare needs are not being met due to a shortage of resources, and in some ways clinicians. 

EldoPath aims to bring 5 critical health services to underserved remote or rural communities through a partnership with health entities: 

a) Gym for the Aged, that would provide a full continuum of health fitness and diet program via a prepaid monthly subscription 

b) Consultation with qualified clinicians/therapist/Community Health Coordinators (drawn from a network of providers from across the US to staff clinics based on predefined clinical needs of health entities (e.g., areas of specialization, schedule, language skills) onsite or online via 2-way video supported by electronic medical records;  

c) Full onsite biomechanics and motor performance diagnostic laboratory equipped with DEXA Scanner capable of different diagnostic investigations,  

d) Dedicated member only mobile app for health information, diet tips, and emergency call. 

e) Responsible referrals to secondary healthcare facilities in public and private sectors 

EldoPath will build and operate clusters of EP units in each partner entities while also creating 12-13 jobs per cluster (mostly for women) for local inhabitants. The mission of EldoPath is to enhance health and productivity of the aged in underserved remote and rural communities and its goal is to transform remote and rural healthcare by demonstrating the commercial viability, long-term sustainability and social impact of the EP model at scale. 

Who is your target end user and why will they be interested? (650 characters or less)

EldoPath's focus is to make sure that elderly patients with the highest level of need get access to fitness and health services where and when makes the most sense for them. The way we aim to do it is to use 'point of care' to bring integrated solutions to the sites where elderly patients go for care. "EldoPath's Points" would also be a very good avenue for the patients to be more friendly with each other by going through routines together and sharing their personal stories with others. One of the things that make Eldopath unique is the way that it would tightly integrate into clinical care settings. This means that we can take care of elderly patients who have the highest level of physical and clinical needs. That’s really our mission, is to go where the need is the greatest to help patients where we can make the most impact.

How is your idea scaleable? (650 characters or less)

EldoPath's strategy ensures the delivery of high-quality fitness and healthcare through a clinically-integrated model, ensuring that our team is fully engaged with their patients’ care team. In order words, this model means that our EldoPath's team are embedded within the healthcare treatment team, becoming an integral part of the team, and utilizing a collaborative care approach. This is a highly scalable idea whereby several entities can decide to set it up in partnership with EldoPath. For instance, a franchising model could be adapted to scale quickly across health systems. Overall, the entire process would create a win-win-win scenario where health entities become more effective in treating patients, gain more traffic to their settings, and patients with the highest level of need get access to health services where and when makes the most sense for them, while clinicians/CHCs are provided with an efficient and cost-effective platform to enhance their practices.

What do/will you measure to know if your solution worked? (500 characters or less)

-Rate of adoption by elderly patients visiting local partner clinics in remote or rural areas -Savings/ROI to clients, -Employment opportunities in the Local Area, -Enhanced productivity of clients, -Rate of early-stage diagnosis of potentially life-threatening conditions

What is the current stage of development of your idea?

  • Prototype: We have done some small tests or experiments with prospective users to continue developing the idea.

  • I want my idea piloted, and I prefer to do my own piloting in collaboration with the health system and with assistance from the Challenge partner
