What challenges or opportunities are you trying to address within the problem? (200 characters or less)

Risk identification, prevention, reduction in cost of care through care coordination, community support for patients. Mobile/audio app to help patients/care agents effectively track care plans/tasks.

Identifying people at risk:

OsteoVault is an online portal supported by a mobile app and an audio device called OV (like Alexa or Google Home). The back-end is a smart AI based rules engine driven by a task management system built around health and lifestyle risk factors associated with Osteoporosis. 

We will introduce this in community and people at certain age can sign up with the help of volunteers or friends using their basic health information. The data will be used against an assessment algorithm e.g. WHO Fracture Risk Assessment tool and identify if a patient is at risk for osteoporosis.

OsteoVault will have built in preventive plans to identify care and lifestyle gaps. Once a patient is identified at risk the OsteoVault's smart system will identify and start to track all associated risk factors e.g. weather conditions and alert user if the rain is going to increase the risk of a fall. The audio device (OV) that can be installed in patient homes, aged care or in any living arrangement will make it easy for old aged patient to interact with OsteoVault's alerts and reminders.

The portal will also have a community module where users can choose insurance plans, ambulance covers and latest offerings related to Osteoporosis. This will also help users connect with other people from the community with similar conditions and learn from their experiences. All these feature will be enabled on audio device (OV) as well, user will be able to hear plan offers and event schedules that are happening in the community. OV will also be able to book clinician appointments when required.

Relatives, care agents, case officers and volunteers will be able to sign-up and view the patient’s plan and track progress (secure authorization mechanism will be used). System will also closely monitor tasks added by care team on the Osteoporosis treatment plan and generate reminders and alerts for all interested parties if a task is missed on the plan. This will make it easier to follow-up with patient actively and not miss any health safety issue.

Alice: OV tell me what is happening today. 

OV: Hi Alice, you have to visit your Osteopath at 11 am. There is going to be rain please wear rubber shoes and avoid slippery surfaces. There is a community event at 3:00 pm in the library for the Osteoporosis awareness. You have two uncompleted tasks from your Osteoporosis management plan, please complete today. 

Intervening for prevention:

Once the patient is placed into an osteoporosis treatment phase, after hospital discharge or early diagnosis from PCP, OsteoVault can be used for care interventions and treatment plan with it's smart task management system. It can be used as a standalone application or as an integrated module with Electronic medical records, health applications and wearable devices using secure channels. Patient’s Osteo care team can add and modify tasks on Osteoporosis Management Plan, communicate with each other, monitor and track patient progress on that plan across multiple settings of care. 

This will help prevent second fracture or any unwanted episode by ensuring close adherence to the plan for patients by using mobile app and OV’s (audio device) alert and reminder feature, coordinated care among multiple care providers and improved quality by continuously tracking the progress. This sharing and accountable care model will also improve the quality of care and reduce the cost for redundant tests, scans and therapies.

Care providers will also be able to share this information with external care teams and care agents using secure messaging infrastructure. 

Key driving factors:

We will run significant campaigns in community and aged homes for awareness. Osteovault will be free for patients to use on mobile and online as website. However funding will be required if low cost audio device installation is required. This model will help drive early adoption by people at risk and push doctors and care agents to get involved.

Care providers will be given free access on the portal to monitor their patient's progress and the system will have open API model where any other health system or device can integrate easily. We will work towards integration with all major wearable devices. I am thinking towards a FHIR based servers for easier interoperability.   

As a long term objective, de-identified data will be share with the insurance companies with a proposal to start an Osteoporosis prevention program for participating clinics. The model will help clinicians to get a share in total reduced cost of osteoporosis treatment if they reach a certain benchmark. This will also help improve the quality of care for osteoporosis patients, reduce cost and inspire care settings to adopt this model. Similar models are already successfully running in US for other chronic disease.

Who is your target end user and why will they be interested? (650 characters or less)

People at risk to get on board with an early identification and help from the system and avoid bone fracture at the first place. Care provider's adoption will be driven by patient adoption. Moreover, OsteoVault will help them track progress easily once the patient is on a Osteoporosis management plan. Osteovault will have options to integrate with their EMR and wearable devices that will save a lot of time spent in data entry and reduce clinical errors. Insurance companies will be interested in data to drive the cost down by offering in-network referral plans, benchmark cost sharing program as explained above, improved and better covers for osteoporosis.

How is your idea scaleable? (650 characters or less)

In this first phase we will start with local communities and adoption by people at risk for early identification, prevention and create awareness for Osteoporosis Second phase will drive adoption by care providers and also integration with different health systems, provider centered design and workflows will make it much easier for care teams to track and monitor progress. On the technical side using a FHIR based API will make it easier for any system to access data from OsteoVault. Third phase will be involving insurance providers where they can use de-identified data from OsteoVault to offer new improved cover plans to patients. Introduce cost sharing models with care providers to drive further adoption for improved quality of care.

What do/will you measure to know if your solution worked? (500 characters or less)

I am inspired by IDEO's work and participated in Design thinking online course by IDEO and enjoyed using those techniques to solve problems in real life. I had this idea for a long time nurturing in my head for chronic care management and after I read the design brief it seems like a very suitable platform to test it.
