What challenges or opportunities are you trying to address within the problem? (200 characters or less)

Doctors treat patients immediate symptoms using patient history. Complete, accurate history is difficult for patients to efficiently, effectively to communicate to multiple providers.

Patient history is often incomplete or inaccurate, yet critical for effective diagnosis and treatment.  Fractures in older adults are common, yet often preventable traumas.  

Patient history, including family history, should help providers recommend diagnostic tests and treatments when prevention is still feasible.  

Patient Health Journal may give complete, accurate history to multiple healthcare providers.  

The journal should be created, updated and reviewed by the patient or patient advocate.  Access to the journal must be maintained per HIPAA privacy rules.  

The journal must be accessible online, via cell phone or email, and protected with appropriate security.

History must include office and hospital visits, symptoms, medicines, procedures, diagnostic tests, test results and providers' notes. 

Who is your target end user and why will they be interested? (650 characters or less)

Target end user is an adult patient or authorized patient advocate. The journal will provide patient history to multiple providers, aiding in effective diagnosis and treatment plans. The journal will be an efficient, effective tool for consistently sharing accurate, complete patient history using convenient, common technology.

How is your idea scaleable? (650 characters or less)

Mobile phones are commonly used by patients and providers around the world. GSMA data reports 8.97 billion cell phone connections, exceeding the current population of 7.71 billion people. 66.53 % of world population owns at least one cell phone. This common technology platform is ideal for sharing information between patients and providers. This solution scales naturally to patients seeking diagnosis and treatment of other conditions.

What do/will you measure to know if your solution worked? (500 characters or less)

Primary success indicators include measured reduction in first fracture rates, measured reduction in repeat fracture rates, patient/provider use rate, and patient provider satisfaction reports from survey.

What is the current stage of development of your idea?

  • I want my idea piloted, but I’d prefer not to be involved in the pilot and prefer that the health system adopts my idea for piloting with assistance from the Challenge partner

I recently retired from ABB, after more than 30 years experience in software development and software team management. I am a "curious designer" and lifelong, problem solver. I have experience sharing or attempting to share my personal medical history (not osteoporosis) with multiple healthcare providers. Office visits are usually limited to around 15 minutes, making complete, accurate history difficult.
