Defining My Own Care
A coordinator to understand what care means to me, and help me make it a reality.
Who is your idea designed for and how does it reimagine the end-of-life experience?
Provide patients a means to discover and make known what they really want, and work with a coordinator to determine how to meet those needs. If it is in spiritual activity or with spiritual advisors of any faith, being around children or animals, recording their story, or even going on a road trip and refusing cancer treatment (for example, Norma in pic). The coordinator can facilitate understanding those needs and their priority, feasibility, and coordinate the team to make it happen.
