With COVID pandemic, US as also many other countries have been suffering from not being prepared timely for addressing the prevention challenges of highly infectious disease. Along with the CDC’s risk screeners and other applications that are now available, solutions and risk models are based on COVID symptoms. In fact, majority of the deaths of COVID have been caused by the existing chronic condition or more such as heart disease, respiratory disease, autoimmune disease or cancer affecting older population by hitting weak immune and respiratory issues.

cliexa recently enabled virtual check-ins for patients with full electronic medical records integration where clinicians can leverage cliexa mobile platform collecting patient reported outcomes and cross-correlate patient demographics, social determinants, pre-existing chronic conditions, medical history, medications, known risks of existing chronic conditions to COVID and connected device data.

What is the need are you focusing on?

Administrative challenges in healthcare services limits clinics to immediately switch to telehealth solution where there is still high risk for patients with infectious diseases as well as their existing chronic conditions. There are 3 parts of this challenge: 1) Providing specialty care to a patient with their existing chronic condition either they have COVID symptoms or not 2) Switch to telemedicine with all administrative and compliance challenges to provide virtual visits instead of in-person visits. 3) Still operate with same patient numbers during this virtual visit shifts as clinics still need to have financial stability regardless of federal and state supported relief funds. cliexa enabled full remote-check ins including clinical assessments for any clinic that can go live with telehealth within days.

Describe the business pivot or adaptation in 3-4 sentences.

cliexa patient onboarding previously has been done via tablets in person in clinics during scheduled visits and enabling patients to be also monitored in between visits using their mobile phones tailored to their subspecialty condition with full integration to EMR's. With COVID, cliexa enabled remote-check ins, enabling new or follow up patients to check-in clinically with screeners and consent forms using their forms where clinicians can then easily initiate a scheduled virtual visits just like they have been doing it in-person. cliexa also enabled CDC guided COVID screening which also covers CDC required data as well as cross-communicate COVID risks with the existing chronic condition. As an example, CDC would have the patient stay home to avoid spreading the disease to recover, where cliexa identifies high risk for a patient with COPD or heart disease that have to be connected to a respiratory support immediately.

Current infectious disease tracking platforms are mostly focusing on symptom data, location, patient demographics and other new data points as applicable which are based on guidelines frequently updated by CDC and other health organizations. Data around each pandemic episode has been collected one off and used as necessary without taking into account the auto-immune issues humans develop over the 50 years caused by environmental effects, new drugs introduced and new treatment methods. cliexa is proposing a mobile tracking platform for patients with one or more chronic conditions to collect demographics, social determinants, pre-existing chronic conditions, medical history, medications, known risks of existing chronic conditions to COVID and connected device data and built a mortality risk prediction algorithm with COVID symptoms. In other words, AI framework would predict the risk for patients with chronic conditions by taking into account their chronic disease activity.

cliexa since it started in 2016 has been focusing on building mobile platforms for patients and their clinicians to track subspecialty specific data in discrete format, enabling clinicians communicate with their patients focusing on thresholds built by their own treatment methods changing across time. cliexa’s first AI framework has been launched at an integrated pain clinic in September 2019, predicting opiate risk by using patient reported outcomes focused on pain, depression and anxiety scales. cliexa predictive opiate risk analysis has shown 97% accuracy compared to clinicians' calculated risk model. We would like to partner with other healthcare organizations, research institutions, non-profit community health organizations to enable and share our solution for better patient-provider communication and patient advocacy.

What do you think are the main barriers to implementing this solution?

1. Crowded digital health market: There are thousands of digital health solutions, mobile platforms that try to solve similar problems and it is extremely hard to find a spot to present what cliexa is capable of. 2. Messaging with digital health market: cliexa is Cisco in 1990's where we are a platform that enables lots of different solutions per subspecialties. It is extremely hard to deliver the layered messaging as there are different target audiences and how they want to hear the story with their priorities. 3. Unfamiliarity with Technology: Our target audiences are good in areas, but to see the big picture, we need visionaries in target audiences who can consume the technical and operational benefits that our platform can provide.

If you are currently working on a pivot, what have you learned so far?

From our implementations with our existing clients, we have seen the adaptation was extremely fast. What helped us to hear what their current challenges were with immediate restrictions COVID provided. We matched our capabilities to address those problems with iterations and our solutions were very-well accepted and we hear great feedback from existing customers.

Welcome to the challenge Mehmet Kazgan  and team. Thank you for sharing the pivot Cliexa has been making in the past few weeks. Could you share some of the learnings you have already seen during the pivots that you have made especially with the remote check-in and screening? Has it been for new clients or for existing ones?
During our past Communication Challenge, I came across this crowdsourced handbook for practitioners - , I wonder if there could be some organizations or people that might be resourceful

Hi Isaac Jumba , great question. We started with our existing clients as Apple and Google blocked all COVID related public apps as you might have heard. That is why we are only allowed to enable the solution for updating our existing solution as cliexa front-end is an iOS/Android native form. We can still partner with a health organization and post a free app, though.
With the outcomes, here is the amazing part. The existing client we have, beginning of March was getting a hit for 80% decline in patient visits because of social distancing initiative. When we enabled the remote-check in, in the beginning of March, they closed March numbers along with telehealth, with 20% increase in patients seen, compared to average in 2019, which is amazing. Not to mention, they also identify COVID risk along with their chronic condition to further advise in their condition remotely. Hope that answers your question and appreciate your comment.
