Who is your idea designed for and how does it better support family caregivers as they care for a loved one with dementia?

Estimates show that 50 million people worldwide are living with dementia. In short order, family members become caregivers, overwhelmed with information and emotional pressure. The role becomes more demanding as dementia progresses, impacting their own health. We provide online, on demand videos by leading experts in organized topics to support caregivers. Your caregiver fit plan is always available, delivered virtually on any device, continually refreshed and curated on your personal dashboard.

The first iteration of the Caregiver Smile Summit launched on October 30, 2017 and is live on the Internet. It is based on a one-time, all access fee. We plan to change it following the models suggested above.


We want people to rock their caregiver experience (like the ladies below), turning it from a burden to an opportunity, from surviving to thriving. Just that change in mindset goes a long way in helping a caregiver through the journey, one where they can savor the moments with a loved one. 

Virtual Summits

Virtual summits are an emerging trend in content delivery. They consist of recorded interviews with top experts in the field that viewers opt-in to watch and which provide excellent information and activities for responsive caregiving.

Virtual Summits happen once or twice a year when our top experts come together to update content or present new topics. This ensures that our content is always current and improving to lift the caregiver experience. All videos deliver rich content, provide holistic action plans and tips for dementia patient care. At the same time, we show family caregivers how to manage themselves, stay fit, engage socially and grow with their roles. 

The Benefit - Short, easy to digest videos that become a baseline primer for education around caregiving.

Choosing the Topics

For the initial Summit, we took a holistic approach to the topics that caregivers wanted to know about and didn’t categorize according to age-related decline or cognitive challenges but did organize from the least acute level - aging in place - to the most acute - hospice, death and dying. 

We also designed the topics so that both the caregiver and the loved one would benefit. We are helping two audiences. 

We will be adding to the current collection of 53 videos with more how-to videos as well as coaching people through troubling moments. They will provide: orientation, preparation, practical and emotional training, monitoring and response protocols, opportunities for respite and ways to spend more meaningful time with a loved one. 

We have laid out six groups of deliverables in our user scenarios below.

Future Iterations

When you review the "Design Innovation Model, Experience Maps" below, you will see that we are suggesting an algorithm-based selection process whereby the consumer puts information about their current situation. Based on that input, we provide them a set of videos for a one-time fee (supported by corporate sponsors) in these areas: 

General Information, Skill Share, Self Care, Fun, Navigating the Health System, Workplace Issues, Housing Issues, Family Dynamics, Financial/Legal, End of Life, After Caregiving.

A consumer has the option to then buy a monthly membership plan. (Though the bigger revenue win is offering the Summit as an Employeee Assistance Program benefit on a per member, per month basis.)

Future topics, updated versions of expert video, activities, reports for science will be made available to yearly subscribers. We also envision being able to take all of the short-listed challenge ideas and create relevant videos that speak to their offerings. For example, the videos from Dance4Healing would make great content on the platform. Also the ideas presented from - Smart Brain Games, Conversation Game, Memory Lane, Memory Music Box, Meaningful Connection through Creative Engagement - all could enjoy exposure through our Summit. We could also work with Partners in Memory to turn written life stories into video stories. And "Prompt by Merica" could offer our videos as a just in time, on-demand solution.

While we would still use the current interview format, we also desire to add more how-to material that helps build skills. 

An idea that surfaced in the first phase of the challenge is allowing caregivers to propose topics and then we find the expert to fill it or we create something new. That will keep the Summit fresh and relevant. A newly introduced private Facebook group could facilitate this. 

The Experts

Because of my own personal network as an influencer in aging, it was fairly easy to find experts. When an expert was not readily identified, we simply searched for the best, reached out and asked. They were gracious to accept.

Value Proposition: Low cost education on a variety of topics that will help you prepare for aging and caregiving. Service matching is done by user managed profile and a survey tool. The basic service package of $59.95 (one time basis) is a fully scaleable/low cost front end solution that provides potential corporate sponsors (like AARP and United) valuable data for marketing and product/service development. 

Subscribers get access to their plan immediately upon payment of their subscription. Upgrade opportunities are presented based on user input. An upgrade to a $119.99 per month is presented with a yearly option to be formatted.

Health Insurers Providers, Agencies, Government

Value Proposition: New caregivers are typically healthy middle-age paying customers when they begin but this changes quickly as they must compromise their work-life balance and dedicate themselves to the caregiver experience. 

Statistics show that after their second year as primary caregivers their health is at higher risk, taxing health systems. Statistics vary but one thing is certain-a significant number of caregivers pre-decease their love one. This number ranges from thirty percent of caregivers to seventy percent of caregivers over seventy.

Early information and support of caregivers to manage their own health and fitness is of primary concern in the coming years, especially when economic pressures add even more to the need for two working adults in families and high costs of care facilities for dementia patients. 

The Summit helps manage unpaid caregiver health. When you give a caregiver information to better manage their life, you reduce stress and disease. This reduces health costs. Provide as a member benefit to reduce costs and increased retention. 

Health Providers 

Value Proposition: Providers need help engaging caregivers whose input is vital to quality and safety. The next iteration can be a bridge to reach caregivers. Better care. Fewer re-admissions. Better PR. Use at discharge planning.

Employers with Caregivers in Workforce 

Value Proposition: Sixty percent of caregivers are in the workforce. Offer the Summit as a value-added benefit to boost morale and increase retention.

For the business market noted above, the Summit along with our other submitted idea - Caregiver Video Support Program - could be a great combination.

How Does It Work / Revenue Stream

Now - It is a simple Woo Commerce purchase. Go to the web site. Select. Pay via credit card or PayPal and instantly access the videos.

How It Could Work - On a consumer basis, we offer one tier of free content supported by corporate sponsors. They can then buy into a monthly membership with an annual subscription discount. 

For business, insurers, health providers, EAP programs, we license it on a per member per month basis to the audiences noted providing feedback loops, audience surveys, and fresh content updated monthly.

We noticed from user metrics of our online videos from the first Smile Summit that users enjoy our holistic approach and chose videos by title and/or expert than by progressive development of dementia.

We carry the videos privately on Vimeo. Vimeo released a report that indicated in the month of November, the first full month of the Summit, our videos were viewed 8,500 times. With 53 videos, that is an average of 160 views per video. With just under 100 attendees, videos were viewed an average of 1.6 times. That indicates that they were of enough value that people wanted to go back and access them. 


A document has been created capturing the feedback from experts and attendees. In certain cases we link to Zoom videos of the feedback session. 

What early, lightweight experiment might you try out in your own community to find out if the idea will meet your expectations?

We have launched the Summit Website and people are signing up and paying. This is a validation of our core offer. A next step would be to show that the model and services work in the business model outlined. We would do a test and survey of a Fortune 500. We see a clear win-win scenario here. Summit can be delivered through employers who make the Summit available as a value-added benefit to caregiver employees. They comprise six of ten workers and 25% are Millennials.

What skills, input, or guidance from the OpenIDEO community would be most helpful in building out or refining your idea?

Design a path forward to prove viability in larger markets. Guidance and plan to do a series of improvements to the current website with diversified products-services in a clear hierarchy and subscription plan. We are getting there. When companies recognize the issue of caregiving in the workforce and use tools like the Summit to educate and support their workforce, they will realize the strategic, revenue and workforce benefits available.

How would you describe this idea while in an elevator with someone?

You're starting to see mom forget. Or she was just diagnosed with dementia. You need advice NOW. Where can you turn for concise, relevant information from trusted experts to navigate your journey? The Caregiver Smile Summit. Video interviews with well-known experts, skill-sharing, self-care, practical advice. Online. All the time. Available from any device.

How does your idea demonstrate our Criteria of Accessibility?

We need organizations. 1. Health Insurers. When you give a caregiver information to better manage their life, you reduce stress and disease. This reduces health costs. Give as a member benefit. 2. Hospitals. They need help engaging caregivers whose input is vital to quality and safety. 3. Employers with caregivers should invest in the Summit as a way to reduce risk, educate in their community and provide a leadership role that shows they are brands that boost people and not just profits.

How does your idea demonstrate or plan to demonstrate scalability?

The technology infrastructure is already in place and because it is an online service, scalability is already built in. Should we work with insurers, hospitals, employers, government, we can private label accordingly. The 53 existing videos are just a start. We plan to add more as we identify topics and experts and also have end user input into topics. Video interviews are time intensive. To scale, I would train others and partner with existing content providers whose material we could license.

How do you plan to measure the impact of your idea?

On a individual purchaser level, we can certainly develop a survey that measure key metrics before watching and after. It gets trickier when you scale. We could work with employers to measure retention and morale among identified caregivers from a baseline. On a broad scale, hospitals can measure PR, readmissions, and other metrics but not all will be attributable to the Summit. We would need to specifically survey and measure health outcomes of caregivers who engage. Same issue with insurers.

What are your immediate next steps after the Challenge?

Frankly, OPENIDEO has encouraged me. With only 100 registrants, the time/cost value had me wandering if it was worth it. This validates the concept. Still I need to consider whether to go back and market the Summit as is or let it stand while I approach the target industries noted above for scalability. I frankly need the validation from this stage to provide proof of concept to giant corporations, who often don't see the direct value (read-revenue) that programs like the Summit bring.

Dear Anthony, I'm excited to know about this resource. It's a wealth of knowledge for both formal and informal caregivers! Considering that financial concerns are front and center for families navigating dementia, I can imagine offering a deal to informal caregivers, such as an X-amount of videos for free, then tiered memberships. Paid viewing of course for formal caregivers! Also, maybe you could add snippets of the video's so people could see if they wish to purchase -much like how iTunes allows you to listen to part of a song before purchasing. Hi Anthony Cirillo - We've gathered a few thoughts around your idea and are curious about your responses.
The Alzheimer's Association has a similar model ( and we noticed it's significantly more expensive than yours! What are your thoughts regarding the sustainability of your pricing model?
The other thing we're curious about is the format if your videos. Are all of them in an interview style? Are any of them specifically about walking through actions? We're inspired by Being in the Moment 's videos and the idea of having positive and negative situational videos. Here are a few for inspiration: (from Jo Schneier )

Joanna The Alzheimer's Association looks like a credentialing module for professional caregivers with the exception of the family series. And that looks like it is aimed at organizations that would use it with caregivers as opposed to caregivers being able to access directly.

So I would expect it to be a higher price but not that high. Selling my program one at a time is unsustainable in supporting the time and effort it takes to produce new content, scale the idea and make it a complete solution. However, let's say I line up an insurer. Let's make it a big one - Kaiser. 11.7 million members. What if they licensed the material? Take the most ridiculous low figure, say a penny per member per month. That comes to $117,000 a month and over $1.4 million a year. That would be a substantial amount of money allowing me to hire a admin/research assistant, keep my web developer, and bring in videographer talent and more.

I watched the videos you suggested and would make a few comments. They are well done and you make a good point about having specific actions to take. We do some of that in our interviews but could do a better job at concrete takeaways. So I would be open to something beyond a one on one interview but that would take bandwidth I don't have as a solo-preneur.

I would also mention a deliberate strategy I chose and that was not to just address Alzheimer's caregivers. We were trying to address any caregiver. For example, I was a caregiver for my mom, who passed at 94 last year. She had all her cognition. My issues were of another variety altogether. Also I want caregivers to understand that they too need to learn what they have to do for their own aging and heed the lessons their experience is imparting. That is why we have sessions on estate planning, advance directives, retirement income planning, etc.

I do want the Summit to be the go-to source because we have already established that we can provide great information and the BEST experts. Read the last entry I made in the feedback loop in which I interviewed one of my experts Dan Ansel. He envisioned this in a way that you hint. If we just concentrate on Alzheimer's for the sake of discussion, we go deep into all the issues a caregiver confronts - aggression, hygiene, depression, etc - I list more in the feedback. We then address each at each stage of the dementia. So you could talk about aggression at early, middle and late stage dementia. Then you can back the interview up with a library of resources. That is why we asked each expert to contribute something to the Summit. But even better, you can then link people to actual products that solve solutions. Dan's company, Active Daily Living, could indeed be a partner in helping us access those products.

Also keep in mind my platform. I connect the dots in healthcare and advocate for people to prepare for aging sooner. The Summit and my other initiatives all complement one another. This is my brand. I am active influencer, good with media, so I am my own best spokesperson.

Since Being in the Moment was mentioned. I thought I would chime in a bit. The link you have above which goes to a training series that is meant to be an instructor led series with a workbook and DVD for home reference. Unless it has changed it is not person centered. In fact, in the description it says this, " Taking care of a person with a dementing illness is specialized work." Perhaps I have not seen an updated version, but it also leans more into TAking charge of the person instead of understanding their viewpoint and looks at problem behaviors rather than actions that are communicating a need to us. Just my .02 Happy MOnday. Karen. p.s. Anthony if you do another Summit let me know. we have met - I don;t know if you remember. I will do it and help you promote it. You just need to get sponsors to do it for free. ; )

Hi Anthony! It looks like the embedded video of Kristi Nelson is working - sorry if you had trouble getting it to embed correctly. Unfortunately we don't have a word version of the PDF, but there are lots of light-lift ways to create a user journey map. Even splitting a piece of paper or word document into a few columns works. In each column can be a picture (either actual or drawn - typically drawn as it's a smaller lift) with description of what's happening.

Kate Rushton In terms of crowdsourcing content, we would face the same dilemma as the project you referred to - "By allowing all types of people to teach on this platform validating individual credentials and abilities is a key challenge." We have already had people self-identify during the first Summit and we will vet them for future initiatives. The growth of the Summit I believe comes from two things. We need corporations to see the value and offer it to employees as part of an EAP program. Also we have talked about chunking some of the content down into other forms and modules to extend the brand and make it saleable Thanks

I think it certainly can work both ways. And when caregivers suggest, it could be topics where we would have to develop expertise and that would add to the body of knowledge.

Kate - I believe this would come from an HR budget. The key is for employers to recognize the morale this will build and the subsequent job satisfaction, retention and productivity it will lead.

I noticed this in your idea submission:
'employers who make the Summit available as a value-added benefit to caregiver employees'. I am not too familiar with the typical employment practices in the US. In the UK, employees are allocated training/professional budgets and this might be included for some employers or budgets for reduced price gym membership. What 'funding pot' could be used?
