Nucleus Innovations Inc aims to apply low cost IVR technology accessible with basic mobile phones to provide caretakers with quick, professional and compelling messages to influence behaviour.

Explain the idea (less than 2,000 characters)

Main challenges to immunization uptake at caretaker demand level are due to demotivation and insufficient knowledge of immunization. This often results to the high immunization drop outs rates and repeated outbreaks of vaccine preventable diseases especially measles. Our idea targets both rural and urban caretakers owning or having access to a basic mobile phone with the aim of promoting healthy behaviors and adherence to the national immunization schedules. Our platform will use basic mobile phones to send short, simple & actionable messages to influence behavior through health tips and adherence reminders. It removes the challenges of remote access by allowing you to follow up on clients any time, anywhere. Immunization service providers are able to work smarter to tailor their services to demand with real-time information. This platform will improve access to quality care, empower and educate caretakers through behavior change, using simple reminders, adherence feedback, motivational messages, symptom management tips and relevant information. Using simple communications like text (SMS) messaging and interactive voice response, allows patients to better manage their health (including the importance of vaccination adherence) and understand what missing means. IVR system is language specific and prompts need to be recorded in the different languages and dialects of specific populations.

Our platform will mainly target East Africa, Uganda. Will target mainly rural caretakers however has application to even urban caretakers.

Who are your end users and how well do you know them? (750 characters)

The end users are caretakers and mothers with infants eligible for immunization. These users are usually previous caretakers at the health facilities and can be followed up using the traditional Village Health Team networks. The assets among the communities include ever increasing accessibility to mobile phones, increased literacy levels, increased penetration of immunisation services to local Health Centre (III) across Uganda. Caretakers (usually previous child bearers) are key determinants to behavior change among new mother.

How is the idea unique? (750 Characters)

Our idea has already proven impact as document by the Call for Life Uganda’s project implemented by Infectious Diseases Institute. Our organisation members have previous experience in implementation of a similar project. Our idea is person centered and promotes user feedback and inquires while providing knowledge through alerts, health worker interactions and auto generated client success stories so as to create positive behaviour change. Our idea uses basic open sourced technology that's making easy to scale. Our ideas provides language specific voice responses and prompts in the different languages and dialects of the user population.

Nucleus Innovations Inc is a unique social enterprise that aims at improving healthcare service delivery though use of modern eco-friendly technologies. Our mission, "Ensuring equitable access to technologies that can improve service delivery and create social enterprise" clearly resonates with our idea of providing ultra low cost Interactive Voice Response technology that empowers and educates millions of caretakers about the significance of immunisation.

Who will work alongside your organization in the project idea? (750 characters)

We shall work along side: Uganda UK Health Alliance, Connect for Life, Local grass root community and health centre leaders, Uganda Immunization Expansion Program on Immunization (UNEPI) - Ministry of Health, UNICEF, GAVI, UNFPA and Makerere University, College of Health Science - Department of Child Health among others.

Dr Tumwine Conrad, Lead Innovator: Medical Doctor and CEO of Nucleus Innovations Inc also working with Mulago Hospital, experience in maternal and new born care. He has previously worked in low health centers and understands the context. Mr Tusubira David, Head Engineer: Currently pursuing MSc in Artificial Inteligence, Head of iLabs Makerere University with experience in IVR systems, RASPBX based Operating Systems and Embedded Systems. His experience is a great asset to the team. Dr Kamurari Solomon, Innovator: Medical Doctor and also Head of Research and Innovations at Uganda UK Health Alliance (UUKHA). The strategic partnership with UUKHA leverages technical and financial support towards transition and scale and other governmental bodies.

Hi @Conrad Tumwine, great reading your idea and understanding how you can leverage on IVR in enabling caregivers access immunization services.

1. Could you share more on the tests/pilot that you carried out and the results or insights from there.
2. How are you able to track the effective of your proposed solution?

It will be interesting for also the team to visualize your solution. The user journey map (shared under the resources section) is a good tool to start with.

Pilot tests done from a Randomised Clinical Trial that did compare the IVR and the normal verbal follow up and reminders among 6000 PLHIV. There was a 62% adherence to HAART among clients registered to the system, these clients also had more knowledge about HIV. Check out more details to this study https://www.idi-makerere.com/publication/call-for-life-uganda-tm-an-rct-using-interactive-voice-response-for-plhiv-on-art-id-3031-2/ The systems' effectivity will be assessed using some of these parameters; 1. Number of facilities and clients registered and using the service. 2. Percentage response and feedback among clients towards scheduled reminders 3. Increased uptake of immunization services 4. Increased community knowledge and attitude towards immunization services. + 5. Reduced outbreaks, morbidity and mortality related to immunisable diseases. These factors combined do assess the direct effect of the system to improving caretakers knowledge, attitude and practices towards utilizing immunisation services. We have also attached the User Journey Map. Please advise accordingly.

Thanks to @Manisha Laroiafor the feedback. Our system employs IVR, the SMS part of this platform is for additional reminders, education messages and feedback from the users. @Tochukwu Egesi idea is also good and i believe we shall in the future use USSD to improve our feedback as currently its not details and we do receive inconclusive data.
