Explain the idea (less than 2,000 characters)

ClinicPak is already leveraging on technology to overcome the limitations of the current paper-based healthcare information system in PHCs in Nigeria by collecting biometric of client including nursing and expectant mothers in PHCs. This present a ready ground for Smart Shot take off as a continuum of care. While a handful of Nigeria women 36% utilize facility based delivery only 14% of newborns received postnatal care within two months of delivery (NDHS, 2013) depriving infant vaccination benefits. Too often health workers especially in PHCs are not supported to improve interpersonal communication skills. Poor communication, and poor quality of care, can significantly hamper routine immunization. As such women who present for antenatal and facility based delivery may be lost to follow up and for post natal care and immunization of their children. Smart Shot immunization tracker will focus on strengthening healthcare systems through improved communication with care givers for immunization and monitoring. With it, nursing, and expectant mothers, are reminded of vaccination date thereby leaving no expectant and nursing mothers’ baby behind.

Kwara State Nigeria. The pictures below are ClinicPak in PHCs across Kwara State overcoming the limitation of paper based healthcare information system wherein Smart Short will be integrated as a continuum of care.

Who are your end users and how well do you know them? (750 characters)

End users are PHCs and Nursing/expectant mothers utilizing PHCs in Kwara State Nigeria. Since 2016 I have worked with Wellbeing Foundation Africa supporting her facility & community based Mamacare program and has benefited over 15,000 nursing and expected mother for improved antenatal and postnatal care. In December 2017, InStrat in collaboration with the Foundation wherein I was among the team officially secured approval from the Kwara State Ministry of Health to deploy CliniPAK to the state. Smart Shot will be fully integrated into the state approved CliniPak. CliniPAK is a tablet computer based application that allows health workers at PHCs to collect client including nursing and expectant mothers health data/biometrics for improved care

How is the idea unique? (750 Characters)

Our idea is unique because Smart Shot will be an integrated model along the continuum of care in the already existing approved Clinipak. This makes it uniquely capable to address the local context of low immunization uptake in the community.

The project will be implemented along with already established key partners like Kwara State Ministry of Health, Primary Health Care Board, Wellbeing Foundation Africa, CliniPak, PHCs for mamacare antennal and postnatal care program of Wellbeing Foundation Africa. For more than 12 years, Wellbeing Foundation Africa has been working on maternal and child health, gender and community development in kwara State, Nigeria and beyond.
