Establishing My Community Health Watch Groups to Support Total Immunization with Monitoring, Evaluation, reporting Reviewing and Mentoring.

To Reduce completely the prevalence of polio and other child hood diseases, We are supporting with the setting up My Community Watch Groups.

Vaccinations are still not fully captured in remote Communities to forestall this we are setting up My Community Watch Group as a panacea to leaving no one behind in Immunization issues, These set group priority is to Monitor, Evaluate and report.

Explain the idea (less than 2,000 characters)

Immunization have been a very Important aspect of Health care, Though there are recorded successes, we are still not yet there in having a hundred percent Success. To ensure a maximum coverage of Immunizations in Remote Villages like Dogongada an outskirt of Abuja in FCT within estimated population of over 2000 inhabitants with viable young populations who need health care, These Communities are also surrounded by other clusters of Community. On our UHC DAY HEALTH Consultations on 12/12/2018, we noticed a huge gap and need to set up a MY COMMUNITY HEALTH WATCH GROUP. The purpose of setting up this group is solely to Monitor, Report and Respond by alerting Immunization Workers to attend promptly to Immunization Issues. The Communities identified to benefit from these interventions are Dogongada, Kabusa, Waru and some minor settlements which all together will account for up to 1,500,000 dwellers. The Case of these Communities are that they lack basic Health Center of close to 200 meters reach. They also have no good road to take pregnant women to hospital early enough for child birth and due to these lapses the Community have cases of high rate of maternal deaths during child bearing. Having noted the plight of the Communities, We worked with them to RAISE THEIR VOICES ON UHC DAY 2018. One of our consultations agreements was to make their case open to the media and that warranted our visit with the media. Our Solution Target is to reach out to these Communities Using their members whom we have clustered into MY COMMUNITY WATCH GROUPS, These groups will Monitor from House to House to seek our new born babies Report these births to Birth Attendants as well as Health Immunization Groups. To Ensure every child is Immunized and they in turn will send reports to us as. These if properly done will bring a 100 percent Immunization coverage and reduce Polio menace. To do these, My health Watch Group will earn honorarium and transports to go round.

These Communities,Dogongada, Kabusa, Waru are Rural Poor Located at the Out Skirtch of Abuja FCT some 25 Kilometers out side the City in North Central Nigeria in Economic Community of West Africa of West African Region in Sub Saharan Part of Africa.

Who are your end users and how well do you know them? (750 characters)

Our End Users are Community dwellers, Pregnant women, Children, Neighborhoods and Citizens who will be privileged to access these health benefits without moving out of their localities. Persons living with disabilities who are pregnant will not suffer much from transportation issues or lack of access to health facilities. Our relationship with them is caring for our fellow citizens who are not as privileged as we are, and who could be of help to us in future by one way or the other. and whose children could become leaders of tomorrow. The Assets and Strengths we see are future developments and wealth Creation through Creating a healthy viable lively Community in the outskirt of Town.

How is the idea unique? (750 Characters)

Our Idea is unique and germane because it has not been tried or tested. Our trail stage will record tremendous research, Monitoring and Evaluation, Reporting and review and Finally adaptation to be replicated in other Communities, Villages and clans. Our Project is a research project that will bring out the How,s and Why,s of what was left behind and will bring up issues for studies and further developments. We believe in the success of our approach as our UHC DAY 2018 Consultations brought up several winning opinions and suggestions from the Communities. Our Value additions will be working with the Community youths and preparing them to take over the leadership. Our expansion plan is to adopt more Villages and Communities and Annex them.

Tell us more about your organization/company (1-2 sentences)

We are a Women Farmers Organization, We farm and emphasize on Mother and child Nutrition and most of our farm produce are used for Nutrition Intervention, Example is OFSP (Orange Fleshed Sweet Potato) We also work on total health as children who are not immunized suffer terribly from Malnutrition. Our Combination of Agriculture Nutrition and Health makes it compulsory that we work directly and with health workers to achieve total Community health coverage which is the UHC DAY focus and program.

Who will work alongside your organization in the project idea? (750 characters)

We are already working with partners like, the health Ministry Vaccination Team. We work with other Civil Society Groups in Health and Agriculture, Nutrition Coalitions, We have a Monitoring and Evaluation Team as well as work with Faith based Organizations. What will come of of our collaborations is that we will build persons who can take up such projects in other localities and are capable of working on their own. An example is the training of Women Rice Farmers in Nembe in Bayelsa State, which was carried out between 2012-2014. After our exit from the job, our existing structure are still strong on ground doing well and training other communities to work without supervision. We did partner with other Non Governmental Organizations.

Our Steering Committee members are made up persons with different professions, Lizzy Igbine Mrs the National President and Chair Person Steering Committee is a graduate of Economics and Management, She has a Fifteen year experience on Management and Twelve years Experience on Agriculture , Nutrition and Heath and she has worked in many capacities. Lilian Okpala is a trained health and Nutrition person and manages out Community- Urban Nutrition activities and trains people on eating healthy diets. Ms Joyce Anikwe is a Mid wife Nurse who works in the AMAC (Abuja Municipzl Area Council) office and an advisor on maternal and child health care programs.Mrs Roseline Hemen is an OND Holder in Nutrition and health and she runs a farm. Our major farm produce are Nuytitious foods and grains example is OFSP (Orange Fleshed Sweet Potato) which we add to families with pregnant women or nursing mothers to adapt as stable foods to help in Fighting malnutrition. We do conduct training's, Workshops.

Overview of How Your Concept Has Evolved (5-6 sentences):

During the expert webinars, I was able to design an Employment questioner and an enquiry letter to find out what is the opinion of our volunteers and to find out if they will like to continue working with us as volunteers based on their perception and acceptance of the volunteer positions. I also learnt to make a pitch, by doing this with the support of my prototype.

Our service works by Community acceptance and partnership. The program is Community owned and by their responses to our questioners eighty percent (80%) affirmed while. On the Vacancy for employment, over 75% were eager to be included in the next batch of volunteers. We did not receive any negative answer.

Feasibility (3-4 sentences):

I received a prototype support by name Gayanjith he supported me with the business canvass development, Drafting the questioner and in my final refinement of our draft budget sheet.

Desirability (3-4 sentences and activity upload):

Our User map is attached and it shows from the first contact or capture to the supervisor and the trail from monitoring and evaluation to review and reporting and there are a lot to learn as we go through these stages. Our Model is attached as in our face book posting link.

Community Focus (2-3 sentences):

The problem is urgent because of several lives of infants lost over the years, The Community have also experienced difficulties in taking their pregnant wives to the maternity clinics on time for delivery and finally the children were mal-nourished due to lack of medical presence. This made the community enthusiastic.

Community Impact (2-3 sentences):

The Community Impact assessment will come with our activities on spaces of land available for Agriculture, Aquaculture, Forest Poultry. We hope to start adding food production, Aquaculture, Poultry and Farmers market to yield incomes and with the Government building a primary school as a result of our UHC DAY 2018 Consultations, the school will be a catchment for selling our farm produce to Government school feeding program and we expect our Government to respond by Partnering with us in the Community Health Watch Group from our activities and reports.

It seems that you already have a lot of connections with the communities so it will make it easier for you to build these community watch groups. I also like your iterative approach which will definitely allow you to learn from the first pilots and refine your idea. I was wondering if you could tell us more about who are the members of these community watch groups and how you are planning to recruit them (and with what criteria). It seems also that these volunteers will be coordinating with health workers and your organization and I'm curious if you've thought of the way you would coordinate the interactions. Dear Lizzy Igbine. I don't have a non-PDF version of the template. However if you open the PDF and go under "Save as Others" you should have the option to Save it as Word. If not, you can simply print and add your sketches and then scan. One last option is to create your own document. The format is not the most important. What matters is to think about all the steps and the questions at each step. Our sketch clearly stated what we met and saw as we approached the community to arrange our consultations and the Future we see as a new city out of the slump. We are ready to partner with persons of good interest to achieve our goal.

Dear Ann yes to your enquiries om Mama Chanjo deals with taping experience of older mothers who have passed through child delivery to tap the knowledge of coping with immunization.
We are also banking on the existing knowledge and support of the communities as we work with them for partnership and quality execution of My Community Health Watch Group. There are similarities and room for partnership.

We are still open to partnerships. We have not concluded on partnerships. The project is at its inception stage and the Communities involved are in expectations of our intervention to help them solve health challenges which have been a source of worry over the years.
