About 5-6% of young children in India have autism, ADHD, learning disorder and speech-language disorders, in isolation or combination. These areresult of a dynamic interaction between biology, family and societal environment.These disorders result in significant parentalstress for long period, several instances resulting in separation and broken families, loss of peace and economy. Thefamily and societal stigmafurther parental stress and risk of disharmony. The therapy are long, resource intensive, requiring several individual stimulation efforts. There is scarcity of competent therapists, structured and comprehensive therapy options. The limited availability, accessibility, standardchallenges make them costlyand less sustainable. The cost and duration of therapy furthers the economic burden on the family. With inadequate therapy, thechild achieves suboptimal cognitive/motor/speechcompetency level, there by limited economic and societal prosperity.
In view of the cost, duration and principle of family engagement, we propose peer family-educator-community partnership model for therapy of these children in resource poor setting like India.For the parents, child is the prime and they can go to any extentin this context. The parents can be coached as co-educators to assist the main therapists as volunteers. They can serve as peer educators for the other parents and carry home the therapy. We hope that these educated parents can assist their peer better, bringing better alignment with socio-cultural and family contexts. The community opinion makers and change agents shall allow changing the perception and creating awareness on the issues. In resource constrained contests with limited trained therapists, the peer family-educator-community partnership model shall allow scaling up of the intervention for these special children to promote mainstreaming. Their mainstreaming shall assist family harmony, peace and prosperity and also impact the societal development.

Who are the beneficiaries? (1,000 characters)

The beneficiaries of the model are:
(1) children aged 9 months to 5 years with autism, ADHD, learning disorder and speech-language disorders, who shall have better chance to achieve functional potential; and
(2) their parents, who shall be trained to serve as peer-educator to support development of these children and peer parents from four area in Odisha (state), India.We expect to benefit at least 400 under-five children with special needs and their 400 parents (at least one per child) through four centers in Odisha.
Apart from these direct beneficiaries, the community at large in these areas shall be benefited through the awareness efforts.
We hope that this shall complement and support the district early intervention centers (DEICs) under the Government of India’s Rastriya Bal Swasthya Karyakram (RBSK) to provide structured and socioculturally appropriate intervention for these children. We hope that this will help several hundred more families with such children in the state

How is your idea unique? (1,000 characters)

Effect of primary and complementary parent-mediated and supported interventions have been documented for such special children, despite the sample size, duration and intensity challenges in the studies. The core interventions (socialization, communication, imitation, play and behavioral skills) require adaptation/revision to the socio-cultural and linguistic context (community and family) in India and for each language context. Our proposal targets at: (1) coaching the parents on generic aspects and one specific skill area to serve as co-educators at the intervention centers and carry over to home; (2) creating parent platform for peer and family support; (3) revision of the therapy module/structure based on the learning and (4) improving community awareness through the opinion makers in these areas. The coached parents shall multiply the educator strength and enable more one-to-one assistance for the children to improve the impact of the interventions, which shall be cost effective.

We have put together a committed team with diverse skill set to achieve the goal. The partners include: (1) Rotary Club of Bhubaneswar: To leverage their strong social presence to improve the community awareness and capacity building; (2) Balkrishna educational and charitable trust – To incorporate customized curriculum for children with developmental needs; (3) Peer parents- Like minded parents are the experienced key resource persons, who will contribute with their immense experience and valuable insights of raising their children; and (4) Technical Consultants including Special Educators, Speech Therapists, Clinical Psychologists, Occupational Therapists, Pediatrician and School Teachers. We hope this diverse partnership shall support the various components of the project planning and implementation.

Please share some of the top strengths identified in the community which your project will serve (500 characters)

Our community is a closely knit community, where people still have the fellow-feeling. In most of the families the father is the bread earner and most of the mothers are educated but home makers by choice. The child is their biggest asset, also the liability. In resource constrained contexts, they are by nature, very creative in dealings and dispositions. So the emotive appeals of such initiatives go a log way to channelize their efforts for peace and prosperity.
