What problem does the idea help to solve and how does your solution work? (2,000 characters maximum)

Indonesia is classified by the World Bank as a lower-middle-income country. Until now, "limited resource" has become the cliché of poor health service for the poor. Although household health insurance population coverage rates have increased in the last decade or so more than 60% of the population still remains without any coverage. Recent estimates indicate that about 18% of its population continues to live below US$1 a day, and about half-lives below $2 a day. Households spend about 2.1% of their total consumption on health. This is exacerbated by the low management system of municipal solid waste, which has been linked to premature deaths, serious illness, and diminished quality of life. On the other hand, households provided with government-run waste management services are forced to pay a monthly collection fee as little as about $1.10 and as much as $3.20.

Malang, East Java, Indonesia. For instance, an urban area like Malang, where this program has been developed, produces more than 55.000 tons of solid waste every day, but only 50-60% is collected. So, it is an opportunity for us to change the behaviour of society with an innovative program that we initiated.

Building Bridges: What bridge does your idea build between people on the move and neighbors towards a shared future of stability and promise? (500 characters)

The insurance system with garbage gives broad access to the community because almost every day, every person, every house produces garbage which is not used so that all citizens can join the program. The active roles and participation of individuals and communities will determine the success of GCI. However, their roles must not be separated from health workers’ endeavours to build health awareness and knowledge.

What human need is your idea solving for? (1,000 characters)

GCI is focused on creating both environmental and health value. The main principle of GCI is organizing a community to create sustainable financing from their own resources in order to improve the access and the quality of public health program from the promotive, preventive, curative and rehabilitative aspects. On the other side, it becomes an incentive for the community to start proper waste management and waste entrepreneurship from the household level. To a certain extent, it also contributes to local sanitation improvement. GCI increases garbage value exponentially, so GCI empowers every individual to mobilize overlooked resources and take an active role in managing health financing. We change the perception and habits of the community to garbage with new innovation system of the garbage insurance scheme. We promotehealth investments with garbage. We want to give freedom from fear, afraid, worry, and pain because we believe, health is a fundamental human right.

What will be different within the community of focus as a result of implementing your idea? (1,000 characters)

By far, there are a number of groups with similar waste collection initiatives, which trade the waste with an amount of money or goods. There is also much of the insurance company in operation. We combine both approaches - exchanging waste into micro insurance and health service. Our distinctive excellences are: 1) using garbage as a source of financing, 2) applying holistic health system, namely health promotion, prevention of illness, treatment for the sick, and rehabilitation after the illness. 3) social entrepreneurship, 4) broad access, a micro-insurance system with this garbage has broad access to the community, 5) community independent financing. Currently, although standard clinics concentrate their efforts on treating the sick people, GCI applies a holistic health system that allows residents to use funds from their garbage production to seek out preventive care in addition to curative or rehabilitative care.

What is the inspiration behind your idea? (1,000 characters)

Garbage is the best solution because almost every day, every house produces garbage which is not used so that all citizens can join our program. For instance, an urban area like Malang, where this program has been developed, produces more than 55.000 tons of solid waste every day, but only 50-60% is collected. Practically, in general, residents must pay fee to the government to dispose of their garbage, but GCI offer residents to submit their garbage worth 10.000 IDR to us, which is used to cover their health care in a holistic manner, cover the patient's treatment (curative), make health care quality improvement programs (health promotion), prevent illness (preventive), and provide rehabilitative health care, such as free treatment, in-clinic counselling, laboratory check, etc.

Describe the dynamics of the community in which the idea is to be implemented. (1,000 characters)

In the societal challenge, our innovation makes the community mobilize their own unused resources to improve health access and breakdown barrier between health facilities and community. Economically, people who are rich or poor to go through insurance schemes with garbage as a premium are possible to get health services without spending more money. Furthermore, in the environmental challenges, our innovation is to make people aware of the environment with not littering to the river or sewers, especially plastic waste. The waste given by community will be managed by GCI to increase the economic value of waste.

How does your idea leverage and empower community strengths and assets to help create an environment for success? (1,000 characters)

In empowering a communal organization, GCI has three steps, namely, building the system, stabilizing, and establishing independence. We always focus on these steps because we have a principle that every program which wants to achieve big implications should have the power of sustainability. To build sustainability, we must build a sustainable financing system. Advocating the leader in the community and community organization approach, which is a strength in both cultural and structural schemes is an initial approach strategy.

What other partners or stakeholders will work alongside you in implementing the idea, if any? (1,000 characters)

Global Development Network (The Global Development Network (GDN) is a public international an organization that supports high quality, policy-oriented, social science research in developing and transition countries to promote better lives) - - We have funded from the GDN around $10000 and invited to India to get mentoring. Department of Environment, in Malang (Department of Environment is an organisation that concerns in environments issue) - - They are supporting events or activities held by GCI, helping to disseminate information to the public to aware about the environment such as inviting people to become members of GCI.

GCI team consists of young people aged 19-25 years with an educational background of college or graduates of bachelor. Teams are divided into Experts Team, Replicators Team, Field Team, and Research and Development Team. Experts Team play a role in quality control, as a consultant concept, and value maintenance of GCI program. Replicators Team role in coordinating the stages of replication GCI with all stakeholders. Field Team carry out the role of technical work in maintenance. GCI. Research and Development Team supervised by the University of Cambridge and the University of Brawijaya in performing the function of development and research programs. This program is expected to have a optimize system of collecting and processing data that facilitates the modification, management, and replication programs
